
Approximately thirty days before the projected
enrollment date, parents will attend a comprehensive
orientation with their school's director. The
session should last approximately 45 minutes
to an hour, and both parents are encouraged to
attend. You will meet all administrative staff
and get introduced to your child's teachers.
The parents and the director will discuss all
elements of Odyssey's operations and Odyssey
will collect all documents required for enrollment,
including:
- Wait List Form
- Annual Enrollment Agreement
- DHR Pre-Enrollment Data
- Contact Information Card
- Alabama Blue Immunization Form (From Your
Family's Physician)
- Child's Social Security Card (Copy)
- Child's Birth Certificate (Copy)
- Mother's Drivers License (Copy)
- Father's Drivers License (Copy)
- Odyssey's Health Policies (Signed)
- Background Information Form
- General Authorizations
- Withdrawal Form
- School Transportation Authorization (If Applicable)
- Internet Video Monitoring Acknowledgement
- Custodial Documentation (If Applicable)
- One Parent Referral - Requested
- One Teacher Referral - Requested
